for Transcript
Any student requiring a transcript of his/her grades must fill out
and submit this form. The King's College is required by law to receive
a signed document from the student requesting the transcript before
the transcript can be sent. If you are requesting your transcript,
please download the form, print it, fill in all fields with accurate
information, and mail the form to the registrar.
Be sure to note any instructions on the form particularly in regard
to fees.
Release of Student Information
This form allows students to permit The King's College to disclose
information such as grades, account balance, etc., to his/her parent(s),
legal guardian(s), company, or another individual. This consent
shall be valid throughout the student’s enrollment, but may
be modified or rescinded in writing by the student. Information
provided to parent(s), legal guardian(s), company, or another individual
is for their use only and should not be disclosed to third parties
without the student’s authorization.
